The Standard Gynaecological Speculum for Routine Adult Examination

The Cusco’s Vaginal Speculum Medium (VM-VSCM) is the most widely used size in the Vital Medical Cusco’s speculum range, designed to accommodate the majority of adult patients presenting for routine gynaecological examination. The 110mm blade length and standard aperture provide clear, stable visualisation of the cervix and endocervix for Pap smear collection — both liquid-based cytology (LBC) and conventional methods — as well as cervical inspection, high vaginal and endocervical swab collection, and intrauterine contraceptive device (IUCD) insertion and removal. The thumb screw locking mechanism holds the blades at the selected aperture without continuous manual pressure, allowing the clinician to complete the procedure with both hands free. The Cusco’s bivalve (duck-bill) design is the international standard for routine gynaecological examination, providing a reproducible, stable view of the cervix when the instrument is correctly sized and prepared for patient comfort before insertion.

Position in the VM-VSC Speculum Range

The medium VM-VSCM is the default first choice for most adult patients presenting for routine cervical screening and gynaecological assessment. When additional aperture is required — typically for multiparous patients or procedures needing maximum cervical exposure — clinicians step up to the VM-VSCL (large, 120mm blade). For nulliparous patients, adolescents, or postmenopausal patients where vaginal atrophy reduces elasticity, the VM-VSCS (small, 100mm blade) is the appropriate starting point. Stocking all three sizes allows clinical staff to select the smallest instrument that provides adequate procedural access — the standard of care for minimising patient discomfort and improving compliance with cervical screening programmes.

Material, Sterilisation, and Availability

The VM-VSCM is manufactured from surgical-grade stainless steel, fully autoclavable at 134°C steam, and suitable for central sterile supply department (CSSD) processing in accordance with local infection prevention and control protocols. The hinged bivalve construction allows thorough cleaning in all joint and pivot areas between sterilisation cycles. The screw mechanism should be inspected regularly and any instrument showing corrosion or mechanical failure removed from service immediately. Available in South Africa from Vital Medical, the VM-VSCM provides clinics, hospitals, and general practices with a reliable, reusable stainless steel speculum for standard gynaecological workloads.
